United Airlines Flight 232 suffered a catastrophic engine failure in 1989 that destroyed all three of the DC-10’s hydraulic systems, leaving the pilots without their normal flight controls. With the aircraft nearly impossible to control, the cockpit crew, assisted by an off-duty flight instructor, discovered that they could influence the aircraft’s direction and descent using only engine thrust. Despite having extremely limited control, the crew managed to guide the crippled aircraft toward Sioux City, Iowa, and attempted an emergency landing. The DC-10 crashed violently, but the crew’s extraordinary teamwork and resourcefulness helped save 185 people who survived the disaster. The investigation later revealed that a microscopic manufacturing defect in a titanium fan disk caused the engine to break apart, sending debris through the aircraft and severing the redundant hydraulic systems. The accident prompted major changes in engine inspection procedures and hydraulic system design to reduce the possibility of a complete loss of flight controls. The disaster remains a powerful example of exceptional cockpit teamwork, ingenuity, and determination under seemingly impossible circumstances.
