Air Midwest Flight 5481 crashed shortly after takeoff from Charlotte, North Carolina, in 2003, killing everyone on board. The disaster was caused by a dangerous combination of maintenance errors and inaccurate weight calculations. Improper maintenance of the elevator control cables severely restricted the aircraft’s ability to respond to the pilots’ commands. At the same time, outdated passenger and baggage weight estimates resulted in the aircraft being significantly more tail-heavy than expected. Once airborne, the pilots were unable to lower the nose enough to regain control. Despite the desperate situation, Captain Katie Leslie fought to keep the aircraft away from a crowded airport hangar, attempting to minimize the potential loss of life on the ground. The crash prompted major safety reforms, including updated federal weight standards and tighter oversight of outsourced maintenance work. For the victims’ families, the tragedy also became a long struggle for accountability, eventually leading to a rare public apology from the airline.
