TAM Airlines Flight 3054 suffered a catastrophic accident while landing at Congonhas Airport in São Paulo, Brazil. The Airbus A320 touched down on a wet runway but failed to decelerate properly because one engine remained at full thrust while the other was in reverse. Under intense pressure and concerned about the wet surface and a disabled thrust reverser, the captain made a critical error during the landing procedure. The aircraft was unable to stop, overran the runway, crossed a major road, and crashed into a nearby building, resulting in 199 deaths. The investigation determined that the crew’s incorrect thrust-control actions were the primary cause, rather than a mechanical failure. However, the airport’s short runway, slippery conditions, lack of adequate runway grooving, and densely populated surroundings left little margin for error and greatly worsened the consequences. The disaster became a stark example of how human error and hazardous operating conditions can combine to produce a catastrophe.
