It happens in the middle of a med pass. In the middle of a conversation with a patient. In the quiet of a shift that is going smoothly enough that your mind has a moment to wander.

I know I am supposed to be doing something else.
Not because you do not love your patients. Not because you are not good at this. You are exceptional at this and you know it. The hard patients thank you at the end of shifts.
