The specific weight of a 20-year prognosis delivered cheerfully, echoing in your head on the London Underground for days. The six months after a diagnosis so bleak and depressive that living and dying became things you could weigh against each other with complete neutrality - and the moment of choosing to live, not because it would be easy, but because there would also be great food, great sex and the possibility of wonder.
