1523-Latent Effector T Cells in Bone Marrow Malignancies

This research identifies a specialized group of tumor-reactive T cells within the bone marrow of patients with multiple myeloma (MM) and acute myeloid leukemia (AML). Unlike the exhausted cells typically found in solid tumors, these bone marrow lymphocytes exist in a latent effector state that remains functionally capable of being activated by immunotherapy. The authors discovered that these immune responses are often driven by noncanonical antigens and shared genetic vulnerabilities across both diseases rather than traditional mutations. To better identify these cells, the team developed TFiT, a transcriptional classifier that successfully predicts how patients will respond to immunotherapy but not standard chemotherapy. These findings suggest that the bone marrow harbors a unique, therapeutically engageable immune compartment that could be harnessed for new cancer treatments. By mapping the antigen landscape, the study provides a framework for developing "off-the-shelf" therapies targeting recurrent immune signatures in blood cancers.
