Monoclonal gammopathy of clinical significance (MGCS) is an overarching classification for non-malignant disorders directly caused by monoclonal proteins produced during clonal plasma cell proliferative disorders. These conditions encompass organ-specific diseases affecting the renal, neurological, cutaneous, and ocular systems, as well as complex multi-system syndromes like POEMS and Schnitzler syndrome. Because diagnosis often requires establishing a precise causal link between the protein and organ damage, clinicians must carefully differentiate true MGCS from coincidental associations. Management strategies typically focus on eradicating the underlying pathogenic clone or limiting the detrimental effects of the monoclonal proteins to halt disease progression.
