In October 2011, 24-year-old Louisa Ioannidis was found dead in Darebin Creek in Reservoir, Melbourne. Her death was ruled non-suspicious, and in 2014 a coroner found it "consistent with drowning" — despite her volatile relationship with her partner and no confirmation the creek was ever deep enough to explain it. For over a decade her family refused to accept the finding, and new evidence, including a pathologist's re-examination pointing to possible neck compression, has since seen the case reopened. This episode looks at how Louisa's death was investigated, what was missed the first time, and why it's taken this long for anyone to ask harder questions.
