Sadly, there are lots of places that fit this description: cities and countries where children are exploited, and where people from all over the world come to participate in horrific acts of abuse. Mike Smith uses his knowledge and skill as a surgeon to get into places where he can help. Along the way, he's had to learn what it means to care for the whole person in these delicate situations.
